The 2026 Joint Provider Surveyor Training is an opportunity for Kansas adult care home providers and surveyors with the Kansas Department of Aging & Disability Services to come together in a cooperative training. This event is coordinated together by LeadingAge Kansas, KHCA/KCAL, KACE and KDADS.

Falls Management Across the Continuum: Prevention, Post-Fall Response, & QAPI Integration

Falls remain one of the most significant clinical, operational, regulatory, and liability challenges across post-acute and long-term care settings. This comprehensive program provides a deep dive into falls prevention and post-fall management through a Quality Assurance and Performance Improvement (QAPI) lens. Participants will engage in case studies, interactive exercises, and expert discussion to strengthen systems, reduce risk, and improve resident outcomes.

By the end of this program, participants will be able to:

Explain neurological and psychological changes associated with aging that increase fall risk.
Identify medication and comorbidity-related contributors to falls.
Implement evidence-based and non-pharmacological interventions.
Reduce unnecessary hospital readmissions following falls.
Conduct effective post-fall reviews and Root Cause Analyses.
Align falls prevention and response programs with QAPI and regulatory expectations.

PART 1: Foundations of Falls Prevention – Risk, Impact, & QAPI Integration: This session establishes the clinical, neurological, psychological, operational, and regulatory significance of falls across skilled nursing facilities (SNF), assisted living (AL), and state-run settings. Participants will leave with practical, evidence-informed strategies to strengthen their falls prevention programs while aligning with regulatory expectations and reducing avoidable hospital transfers.

Key Topics include: Scope and Impact of Falls, Physiological & Psychological Changes with Age, Medication-Related Risk Factors, Falls Prevention Strategies, and Components of a Strong Falls Prevention Program.

PART 2: Post-Fall Processes – Standards, Response, & System Evaluation: This session moves beyond “what to do after a fall” to focus on how to evaluate and improve systems following an incident.

Key Topics include: Standards of Practice for Post-Fall Response, Immediate Assessment and Neurological Monitoring, Injury Evaluation and Documentation, Communication Expectations (provider, family, team), Clinical Decision-Making to Prevent Unnecessary Hospital Transfers, Structured Post-Fall Review Tools, Recognizing When a Fall Signals System Gaps, Integrating Findings into QAPI

PART 3: Root Cause Analysis (RCA) – Looking Beyond the Incident: Effective Root Cause Analysis (RCA) requires structured critical thinking and systems evaluation.

Key Topics include: Conducting a Meaningful RCA After a Fall, Identifying True Root Causes vs. Surface-Level Factors, Evaluating Impact on Other Residents, Trends Analysis and Pattern Recognition, Linking RCA Findings to QAPI Initiatives, Sustainable Corrective Action Planning, Avoiding “Checkbox Compliance”

PART 4: Moderated Panel Discussion – Perspectives from the Field: The program concludes with a moderated panel featuring perspectives from a Skilled Nursing Facility provider, an Assisted Living provider, SNF surveyor, and a State-Licensed Only surveyor. The panel discussion will focus on falls prevention expectations from regulatory and provider perspectives, documentation and system process expectations, managing high-risk residents, avoiding unnecessary readmissions, case studies and real-world scenarios, and practical lessons learned. Participants will have the opportunity to ask questions, explore gray areas, and gain clarity on compliance expectations and quality improvement strategies.

Continuing Education

This educational event has been approved for up to 6.5 continuing education hours in the core of Administration for Licensed Adult Care Home Administrators, Adult Care Home Operators and Dietitians by the Kansas Department of Aging and Disability Services. Health Occupations and Credentialing approved provider number (LTS A0009).

LeadingAge Kansas is approved as a provider of continuing nursing education by the Kansas State Board of Nursing. This course offering is approved for 6.5 contact hours applicable for APRN, RN, LPN relicensure. Kansas State Board of Nursing provider number: LT0102-1116.

All other disciplines will receive a certification of attendance.

Substitutions for events are always welcome. These individuals can be anyone employed on your campus, board members, resident family members, or residents. Substitutes must be made for the whole event. In certain circumstances, if substitution, live stream, or video alternative is not an option, a refund may be provided based on the guidelines below.
